New Patient Registration

To register please either bring in your medical card or complete a registration form and new patient questionnaire available to download below or from reception. Please bring into the surgery two forms of identification. One must be a photo ID for instance passport or driving licence and the second must be a utility bill or solicitors letter in your name at your current address.

All newly registered patients will be asked to complete a questionnaire.

NB: You will need to bring in 1 photo ID and 1 bill showing your name and current address to register for online services.

Medical treatment is available from the date of registration.

Change of Address

Please inform the receptionist if you change your name, marital status, address or telephone number so that we can keep our records accurate.

Please note: If you move address, please check with the surgery that you are still within the practice boundary.


If you are on repeat medication, please bring a printout from your previous surgery when you register.  You can be issued with one month of your medication but will then need to see a doctor before you are issued with a repeat prescription.  Please allow 48 hours for your prescription to be prepared (longer if you want it to be sent to a pharmacy).

Access for The Disabled

We are fully equipped with facilities for the disabled with access ramps, wide doors and a toilet.

Registration Forms

Please note there are two parts to the registration forms. All registrations require a part one to be filled out and then a part two according to age. If you are 16 and above please fill out the Adult part two. If 15 or below then please fill out the Children's part two.


We will be unable to register you if we do not receive both parts of the registration forms.
